Diabetes Care Plan
The Care Plan lets you see how the things you do to manage your diabetes, such as being active, eating healthy foods, and taking your medication work to keep your diabetes under control.
You will be able to set both daily and long term goals, and to track your progress against those goals.
You can set daily goals for:
- Checking blood glucose (number of times per day)
- Blood pressure
- Physical activity level
- Weight
You can set long-term management goals for:
- A1C
- Cholesterol
- Triglycerides
You can also enter goal dates for follow-up doctor's appointments.
