Diabetes Care Plan
The Diabetes Care Plan lets you see how the steps you take to manage your diabetes, such as being active, eating healthy foods, and taking your medicine, work to help keep your blood sugar in control.
You will be able to set both daily and long-term goals, and you’ll be able to track your progress toward meeting those goals.
You can set daily goals for:
- Checking blood sugar (number of times a day)
- Blood pressure
- Physical activity
- Weight
You can set long-term goals for:
- A1C
- Cholesterol and blood fat levels
- Triglycerides
- Weight
You can also enter goal dates for follow-up appointments with your diabetes care team.
