Diabetes can affect many areas of your health. That’s why it’s a good idea to have a diabetes care team working with you.
The role of your care team is to make sure that you receive the best and most current diabetes care. Each member of your care team is important. But the most important person on your diabetes care team is you. Your role is to make sure that your care plan meets your needs and that you can follow it. Together, you and your team will work to adjust your diabetes care plan so that it fits your needs and preferences.
